Can Philadelphia CBH Cover an LSW or Unlicensed Mobile Therapist?

By George Ruan • October 30, 2026

Yes, through specific pathways. Community Behavioral Health (CBH) in Philadelphia permits an LSW to be credentialed and contracted in a qualifying group practice with documented clinical supervision. Its June 16, 2026 notice says an LSW may not be credentialed or provide services as an independent practitioner. Separately, CBH’s IBHS reimbursement bulletin names licensed and unlicensed mobile-therapy services. Neither pathway permits any unlicensed person to bill general outpatient psychotherapy under a supervisor.

This article concerns CBH’s Philadelphia behavioral health arrangements, using its June 2026 LSW notice and IBHS bulletins together with Pennsylvania’s adopted IBHS staff and supervision rules, retrieved October 30, 2026. Group-practice LSW credentialing and an IBHS agency’s mobile-therapy service are distinct settings. Do not transfer their conditions or codes between settings.

What does an LSW need for CBH group-practice participation?

CBH’s June notice requires a completed clinical practicum or internship within the LSW’s MSW or DSW education, accurately reflected in CAQH. The group must include a clinically credentialed LCSW, LPC or LMFT. That clinician must supervise the LSW, sign off on documentation and take responsibility for services to CBH members. The LSW’s HR file must contain a supervision agreement with one of those clinically licensed practitioners in the group.

Does the group’s contract automatically cover the LSW?

No. The notice describes an LSW who may be credentialed and contracted as a practitioner in the group under its stated conditions. It does not substitute a group’s existing contract for completing the individual’s applicable process. Nor does it publish a universal rendering-NPI instruction or a supervisor modifier. Use CBH’s applicable claim instructions for the approved practitioner and service rather than inventing a supervisor-NPI substitution.

Can an unlicensed clinician deliver IBHS mobile therapy?

Yes, if the individual qualifies under the specific IBHS mobile-therapy staff rule and the agency’s approved service arrangement. Pennsylvania section 5240.71(c) includes routes beyond independent licensure: specified graduate degrees with clinical coursework plus either qualifying direct-service experience or a qualifying clinical or mental-health practicum. An unfinished degree or being called an intern is not, by itself, one of these graduate-level routes.

Which mobile-therapy qualifications does the state rule recognize?

Section 5240.71(c) names licensed psychologists, LPCs, LMFTs and LCSWs; it also includes an LSW or behavior specialist with the specified graduate degree and clinical or mental-health direct-service practicum. Other routes require a graduate degree with at least nine credits in clinical practice in the specified disciplines, plus either at least one year of full-time direct mental-health service experience with children, youth or young adults, or a qualifying clinical or mental-health practicum. Read the exact degree, accreditation and discipline terms before qualifying a staff member. Behavior consultation and autism-specific qualifications are separate provisions.

What does CBH’s IBHS billing bulletin actually support?

Bulletin 25-08, effective April 1, 2025, lists H2019 for unlicensed mobile therapy and H2019 U9 for licensed mobile therapy in the listed regionalized IBHS assessment and initial-treatment levels of care. It includes separate FQ variants. Those rows establish a distinction between staff categories within that program; they do not authorize H2019 for ordinary office psychotherapy or make FQ an unlicensed-staff modifier. Use the current authorized level of care, applicable modality instructions and contracted Schedule A rather than importing a rate from another service.

What supervision is required for IBHS mobile therapists?

Section 5240.72 requires supervision by someone meeting the clinical-director qualifications. Its mobile-therapy and behavior-consultation requirements include at least one hour of individual face-to-face supervision monthly and at least 30 minutes of direct observation every six months. Additional supervision applies to staff who supervise BHTs. The rule allows the specified live two-way video approach, requires documented supervision, and requires supervisor availability during service hours, including evenings and weekends. These requirements do not replace the different group-practice conditions in CBH’s LSW notice.

Have later IBHS requirements changed?

Yes. Bulletin 25-24 changes program and authorization requirements from September 2025, including requiring at least two family goals in the treatment plan for authorization. It also changes BHT daily-unit rules from October 2025. This article does not treat the earlier staff-code table as the entire current IBHS authorization policy. Confirm the current approved service, treatment plan, authorization and agency contract before using a code.

What remains unresolved by these sources?

These sources do not establish a blanket CBH pathway for every LAPC, LAMFT, student or other unlicensed clinician in general outpatient practice. They also do not provide a universal supervisor rendering-NPI rule. An LSW group pathway and a qualified graduate’s IBHS mobile-therapy pathway are supported answers; other clinician and setting combinations require their own applicable authority.

Frequently Asked Questions

Can a Pennsylvania LSW practice independently with CBH?

No. CBH’s June 16, 2026 notice excludes independent LSW practice but permits credentialing and contracting in a group under the specified education, supervision and documentation conditions.

Can an LPC supervise the LSW in the CBH group pathway?

CBH’s notice names a clinically credentialed LCSW, LPC or LMFT in the group, with supervision, documentation sign-off, responsibility for services and a written supervision agreement.

Does CBH reimburse unlicensed mobile therapy?

Its IBHS bulletin lists an unlicensed mobile-therapy category in specified levels of care. The individual must qualify for that IBHS role; this is not a blanket authorization for unlicensed outpatient psychotherapy.
