Can Licensure Candidates Provide SoonerCare Psychotherapy?

By Bomi Health • October 30, 2026

Yes, within the eligible outpatient behavioral-health program. OHCA’s psychotherapy rule expressly permits an LBHP or licensure candidate to deliver individual, group and family psychotherapy. The candidate must qualify under the staff-credentials rule, work through an eligible enrolled provider arrangement and meet the service-specific requirements. This SoonerCare answer is separate from Oklahoma HealthChoice.

Which candidate professions qualify?

OHCA defines candidates as practitioners actively and regularly receiving board-approved supervision toward licensure in psychology, clinical social work, professional counseling, marriage and family therapy, behavioral practice or alcohol and drug counseling. Supervision must continue with a fully licensed clinician if the board’s hours are complete but licensure has not yet been obtained.

A candidate for professional counseling or marriage and family therapy therefore has a documented pathway, but the title “intern” alone does not establish candidate status. The rule is not blanket permission for every student or unlicensed graduate. Board approval and actual scope must be established.

What must the supervisor do?

The responsible licensed behavioral health professional must review the case with the candidate, be personally available or ensure an LBHP is available for consultation while services occur, agree with the service plan and confirm that the service was appropriate. The medical record must document compliance; the responsible LBHP must review, countersign and date the service plan and its updates.

These provisions describe consultation and oversight. They do not say the supervisor must sit in every therapy session. Do not import a different insurer’s room-presence or supervisor-NPI rule into SoonerCare.

Agency enrollment and candidate enrollment are separate

The cited outpatient program requires an accredited or ODMHSAS-certified organization with an OHCA contract. OHCA’s participation rule requires contracts for both the agency and its individual rendering providers, plus site-specific arrangements. Independent professionals and other clinic types are addressed in separate rules; an ordinary private practice cannot assume it qualifies simply by hiring a candidate.

The rule identifies the candidate as an individually contracting rendering provider. The reviewed provisions do not specify the exact CMS-1500 NPI box or modifier combination for every candidate claim. Use the actual enrolled agency’s current claim instructions to resolve those fields; a supervisor’s co-signature is not evidence that the supervisor should replace the performer on the claim.

Which psychotherapy conditions still apply?

Individual psychotherapy must be goal-directed and tied to the member’s service plan, in a confidential setting. The rule specifies documentation and daily/weekly limits. Individual and group psychotherapy are not reimbursable for children under 36 months under these provisions; medically necessary family therapy has a separate young-child pathway and prior-authorization criteria.

Group size, family consent and treatment-goal requirements remain service-specific. The agency rules do not independently prove a particular SoonerSelect managed-care contract’s claim handling, or permission for an unrelated school, facility or commercial product. This review uses OHCA’s published staff and psychotherapy rules revised September 1, 2024, together with its provider-participation requirements.

Frequently Asked Questions

Can an LPC candidate provide SoonerCare psychotherapy?

A professional-counseling licensure candidate can qualify under OHCA’s outpatient program when board-approved supervision, individual contracting, provider setting and service requirements are met.

Is SoonerCare the same as HealthChoice for supervised billing?

No. They are different payer programs. A HealthChoice supervision or claim rule does not establish SoonerCare permission.

Does a graduate qualify automatically?

No. The person must meet the actual licensure-candidate definition and the relevant enrollment and supervision requirements.
