North Dakota Medicaid: Can LAPCs and LBSWs Bill Under Supervision?

By Bomi Health • October 30, 2026

Yes, for specified behavioral health rehabilitative services—not a blanket psychotherapy allowance. North Dakota Medicaid’s March 2026 policy includes licensed associate professional counselors (LAPCs) and licensed baccalaureate social workers (LBSWs). The individual must qualify for the particular service and work for an entity with a Medicaid provider agreement. Both the billing and servicing NPIs must be enrolled on the service date.

Does supervision mean using the supervisor’s NPI?

No. The policy expressly prohibits these rehabilitation practitioners from billing under a supervising practitioner’s NPI. Required clinical supervision still applies; it does not replace the identity of the person who delivered the service. The claim also needs the ordering, referring or prescribing practitioner’s individual NPI. Those are different roles.

Which services can an LAPC or LBSW provide?

Use the policy’s service-by-provider matrix, not the license name alone. For example, its H0002 screening, triage and referral row includes both LAPCs and LBSWs; its H0031 behavioral-assessment row includes LAPCs but not LBSWs. Each service also has its own definition, limits and documentation. A rehabilitation code cannot be substituted for a psychotherapy visit merely because the clinician is supervised.

Do unlicensed students qualify?

Student status alone is not a provider category in this policy. It separately defines behavioral modification specialists, behavioral analysts and mental-health/behavioral-health technicians with their own education, experience, certification or supervision requirements. A person must meet an actual listed category and its service limits; an unfinished degree or a supervisor’s signature is not enough.

Treatment plans, telehealth and claim submission

The rehabilitation pathway requires medical necessity, an appropriate licensed-practitioner recommendation and a plan directed toward restoring functioning. The matrix marks particular services as telehealth-eligible; that is not permission to deliver every rehabilitation service remotely. Submit the covered service on a CMS-1500 or 837P using its applicable coding rules. This article does not establish CPT psychotherapy eligibility, commercial coverage or a separate managed-care contract.

Frequently Asked Questions

Can an LAPC use a supervisor’s NPI for ND rehabilitation claims?

No. The March 2026 rehabilitation policy requires eligible enrolled servicing practitioners and prohibits billing these services under a supervising practitioner’s NPI.

Does this establish coverage for an LBSW’s psychotherapy visits?

No. This is a service-specific rehabilitation pathway, not a general psychotherapy permission.
