Does BCBSND Allow LAMFT and Psychology Trainee Billing?

Can a LAMFT bill BCBSND under a supervisor today?

Yes, under the current provider manual’s professionals-in-training pathway. Page 53 explicitly lists a Licensed Associate Marriage Family Therapist (LAMFT), a postdoctoral psychology resident and a master’s-level psychologist among eligible professionals whose services must be billed using the supervising provider’s NPI. That is a narrower and more useful answer than treating the manual’s general no-incident-to statement as a ban on all supervised therapy.

The trainee must be participating in a supervision plan toward becoming a BCBSND-recognized payable provider. Their supervisor must satisfy the applicable board’s approval requirements, hold the relevant state license, registration or certification, and meet BCBSND credentialing criteria. Direct supervision requires presence in the office suite and/or on the virtual-care visit, with immediate availability for assistance or direction.

Which other therapy trainees are named?

The same section includes a psychiatry resident in psychotherapy training under a psychologist’s direct supervision. It separately identifies qualifying psychiatric physician residents from their second year onward who meet accrediting-body supervision standards. Those are specific categories, not a general graduate-student exception.

The current list does not establish a general supervised-billing pathway for every social-work or counseling associate, undergraduate intern or master’s student. A master’s-level psychologist is a named provider category; it does not mean any person studying for a master’s degree. An unlisted credential remains unestablished by this section rather than automatically allowed. This article does not transfer these commercial-manual findings to Medicaid Expansion, Medicare or another Blue plan.

Why does the manual also say no incident-to billing?

Page 55 says BCBSND does not recognize Medicare-style incident-to billing. Page 53 separately specifies eligible professionals in training and the supervisor-NPI protocol. Read those provisions together: ordinary incident-to billing is not a substitute for satisfying the named trainee pathway.

The current trainee section supplies the supervising-NPI instruction but does not itself prescribe SA for that pathway. Do not import the SA requirement from the future policy below into current claims solely because the service is supervised.

What changes on January 1, 2027?

BCBSND has published Adm 148, Incident To Services, with an effective date of January 1, 2027 and an applicability label of Migrated Commercial. It is future guidance as of this October 30, 2026 review. The provider manual says migration occurs in phases through the end of 2027, so the calendar date alone does not establish that a member has migrated.

For psychological behavioral-health services, the future policy identifies master’s- and doctorate-prepared clinicians registered with their state board and on a formal licensure track, plus LMSWs subject to state requirements. That wording does not establish current October 2026 permission or automatically map every North Dakota license title to a future eligible category.

What does the future behavioral-health pathway require?

Adm 148 requires the supervisor and associate to be in the same provider group, the supervisor to monitor treatment and be available by telephone or telehealth for consultation, review of overall treatment documentation, and documentation of supervision meetings. It specifies the supervisor’s name and identification on the claim and SA on the service line.

The supervisor’s education must be at least equal to the associate’s. Its example supervisor professions are subject to state licensing requirements; the list does not override state-law pairing restrictions. Associates cannot supervise other associates or practice independently under this provision. These are the policy’s behavioral-health general-supervision conditions; its separate palliative-care direct-supervision paragraph should not be substituted for them.

Which rule should a practice use?

For a current LAMFT or named psychology trainee, start with the current professionals-in-training requirements and supervisor-NPI instruction. For 2027 services, first identify migrated versus non-migrated commercial coverage, then use the applicable policy, clinician category and supervision requirements. Do not treat a web-page tab labeled Current as overriding the printed January 2027 effective date.

Frequently Asked Questions

Can a LAMFT use a supervisor NPI with BCBSND now?

Yes, when the current manual’s formal training, board approval, supervisor credentialing and direct-supervision requirements are met.

Is BCBSND’s January 2027 associate policy already effective?

No. Adm 148 states January 1, 2027 and applies to Migrated Commercial. Migration occurs in phases; it is not permission for current October 2026 claims.

Does every BCBSND supervised therapy claim need SA?

The future migrated-commercial Adm 148 behavioral-health pathway requires SA. The current professionals-in-training section does not itself prescribe SA; do not apply a future rule early.
