Can Associates Bill Trillium Tailored Plan and Medicaid Direct?

Does Trillium cover licensed-associate outpatient therapy?

There is a conditional licensed-associate pathway. Trillium’s provider manual effective October 1, 2026 lists NC Medicaid policy 8C as a reference for covered outpatient behavioral health. Policy 8C permits the listed associate professionals to bill eligible outpatient services when enrollment, scope, service and plan requirements are satisfied. Trillium also publishes a current 2026–2027 Medicaid outpatient benefit plan, revised July 8, 2026, that cites 8C for individual, group and family therapy. The Medicaid Direct pathway is addressed separately below. This concerns Trillium Health Resources in North Carolina, not Trillium’s Oregon plans.

Which associate licenses does the policy name?

Section 6.1 includes licensed clinical mental health counselor associates (LCMHCAs), licensed clinical social worker associates (LCSWAs), licensed marriage and family therapist associates (LMFTAs), and licensed clinical addiction specialist associates (LCASAs). Licensed psychological associates (LPAs) are also listed. Attachment A assigns provider categories to services; a listed credential does not authorize every procedure. The psychotherapy tables include the associate categories, subject to their clinical scope.

Whose NPI is used: the associate’s or the supervisor’s?

Policy 8C requires the individual licensed professional to enroll directly with Medicaid and have their own Medicaid provider number and NPI. It expressly prohibits billing incident-to a physician or another licensed professional. Required clinical supervision therefore does not substitute the supervisor’s enrollment for the associate’s. This pathway does not establish permission for an unlicensed practicum student or an unregistered graduate.

Does an associate need a separate service order?

Yes. Section 5.3.1.4 requires an order from a physician, doctorate-level licensed psychologist, nurse practitioner or physician assistant before or on the first treatment date, excluding the initial assessment. A clinical supervision agreement and a service order serve different purposes. The policy includes a separately documented urgent/emergent verbal-order process; ordinary supervision alone is not that exception.

What still depends on Trillium and the member’s plan?

The clinician must meet Medicaid participation requirements and applicable Trillium contracting, credentialing and service requirements. Medical necessity, member eligibility, code-specific limits and any applicable authorization requirements still matter. The payer’s policy reference is not proof that a particular clinician is in network or that a claim will be paid. State-funded services and other plans require their own benefit and provider rules; a shared administrator or claim identifier does not transfer Medicaid permission.

What changes for Trillium Medicaid Direct?

For Trillium-administered Medicaid Direct outpatient behavioral health, the current Medicaid outpatient benefit plan supplies the payer-specific reference to 8C. Its individual-therapy table lists 90832, 90834 and 90837 and cites that state policy. The licensed-professional enrollment, own-NPI and no-incident-to conditions therefore remain central; a supervisor’s license does not replace the associate’s enrollment. Member eligibility, the applicable service table, medical necessity, documentation and Trillium participation requirements still apply.

Keep the programs separate when identifying the member’s coverage. Trillium’s claims protocol effective October 1, 2026 separates Tailored Plan claims, identified there by 43072, from Medicaid Direct behavioral-health and state-funded claims, for which it lists 56089 in one transaction channel. Other channels use different identifiers. These numbers identify program distinctions, not permission or submission instructions. The shared 56089 entry does not extend the Medicaid outpatient benefit to state-funded recipients.

Which sources and dates support this answer?

Reviewed October 30, 2026. The currently downloaded Trillium manual is effective October 1, 2026; older search snippets describe a July 2025 version. The freshly retrieved state policy 8C retains its January 1, 2025 amended date. A proposed revision or comment notice is not an adopted replacement. The current benefit index links the 2026–2027 Medicaid outpatient plan revised July 8, 2026; older search snippets label the same URL 2025–2026. The October 1, 2026 claims-system change is a routing date, not proof that associate eligibility began on that date.
