Can LMSWs and LCATs Enroll With Univera Under Supervision?
By Bomi Health • October 30, 2026
Yes, Univera Healthcare’s current enrollment page expressly includes Licensed Master Social Workers (LMSWs) and Licensed Creative Arts Therapists (LCATs) in its supervised non-physician enrollment pathway. That is a meaningful participation option, not a blanket approval to bill every supervised therapy session. The clinician must complete the applicable enrollment process, and the member’s benefit and the accepted supervisory arrangement still matter.

What does the current enrollment pathway require?
Univera’s Join Our Network page lists both LMSWs and LCATs under its Application for Non-Physician Health Care Practitioner. The linked form, UN-3352 revised December 2025, asks for the clinician’s individual NPI, license and state, practice details, group information and tax identification number. It also requests the collaborating provider’s details and attestation. LMSWs and LCATs must include the collaborator’s license along with their application materials.
The current page requires an accessible physical practice location within the plan’s service area and says telehealth-only providers will not be credentialed or contracted. The application likewise attests to maintaining a physical location. An application is not an approval: the page says a request to participate does not guarantee a contract and describes when contract and credentialing requirements apply.
Who may supervise an LMSW or LCAT?
There is an unresolved wording issue in the public sources. Univera’s September 2025 manual, pages 3–16 and 3–17, describes a written collaborative arrangement for mental-health services such as psychotherapy. It names a participating LCSW with an R designation, a psychologist identified as PhD, or a psychiatrist identified as MD. The newer application uses generic collaborating-physician wording rather than restating that list.
Do not treat the older R designation as a current New York licensure requirement. New York’s Office of the Professions says the separate R insurance privilege requirement was eliminated effective January 1, 2023. That state change does not, by itself, establish how Univera currently applies its supervisor-registration criteria. Nor does the generic physician label on the form prove that all non-physician collaborators are excluded. Confirm the proposed collaborator’s acceptance rather than silently expanding or narrowing the payer’s categories.
Does enrollment mean using the supervisor’s NPI?
No such substitution is established by the current enrollment page or form. The form asks for the clinician’s individual NPI as well as collaborator and group information. The September 2025 manual separately describes an individual NPI and registration under the supervisor’s tax identification number. Sharing a tax ID is not the same as replacing the treating clinician’s NPI on a claim. The reviewed sources do not resolve a complete rendering-NPI or modifier instruction for this pathway.
Does this include students, other associates or every product?
The current pathway specifically names LMSWs and LCATs, both licensed categories. It does not establish enrollment or reimbursement for an unlicensed student, every counseling associate or every other supervised designation. LCAT enrollment is also not evidence that LCATs are merely unlicensed trainees.
This is a Univera participation answer, not a Medicare coverage determination or a permission rule for every product using the same payer name. The form contains product-enrollment fields and exceptions, but those alone do not establish a Medicare or Medicaid benefit. Verify the actual member’s product, the clinician’s accepted participation and the covered service.
Frequently Asked Questions
Can an LMSW enroll with Univera under supervision?
Yes. Univera’s current enrollment page explicitly lists LMSWs in its supervised non-physician pathway. Enrollment approval, the accepted collaborator and the member’s benefits still need to be established.
Does Univera also list LCATs?
Yes. Licensed Creative Arts Therapists are expressly listed on the current page and December 2025 non-physician application. This does not establish a blanket rule for students or other associate credentials.
Should the supervisor’s NPI replace the therapist’s NPI?
The reviewed enrollment sources do not establish that substitution. They request individual, collaborator and group information; the older manual’s shared-tax-ID instruction is not a complete rendering-NPI rule.
Sources and review limits
Reviewed October 30, 2026. The live enrollment page and its December 2025 application establish the current named enrollment option. The September 2025 manual is retained public guidance used here only with its date disclosed; current adoption of every manual detail was not independently established. Its supervision and tax-ID language therefore should not be converted into an unconditional current claim recipe.
