Does Nebraska Medicaid Allow PLMHPs to Provide Individual Therapy?

Yes: the current service definition lists PLMHPs

Nebraska Medicaid's currently linked Adult Outpatient Individual Therapy / Youth Mental Health and Substance Use Disorder service definition explicitly includes provisionally licensed mental health practitioners (PLMHPs) and provisionally licensed psychologists. Services must remain within scope and receive the supervision required by the practitioner's license. This is a positive, service-specific staffing answer, not merely a statement that a provisional license exists.

Reviewed October 30, 2026 against the Medicaid Behavioral Health Service Definitions page and its linked individual-therapy definition. That page still describes the broader replacement provider manual as under review. Draft replacement language is not used here as adopted billing authority.

Which services and clinicians does this cover?

The definition covers outpatient individual psychotherapy, with services in an appropriate office, clinic, home or other environment. Its staffing list includes PLMHPs and provisionally licensed psychologists alongside physicians, PAs, APRNs, psychologists, LIMHPs and LMHPs.

It separately adds LADCs and provisional LADCs for youth substance-use-disorder services only. That addition does not give a provisional alcohol/drug counselor permission to deliver general mental-health psychotherapy. Nor does this staffing list establish permission for unlicensed counseling students or every person holding a provisional social-work certificate.

A PLMHP is not an independently licensed mental health practitioner. Hiring and enrollment should use the actual credential, not an abbreviation that obscures the provisional status.

What supervision and treatment documentation are required?

The individual-therapy definition requires an initial diagnostic interview before mental-health treatment, an individualized treatment plan and review at least every 90 days or more often when indicated. Any supervising practitioner's involvement must appear in the initial diagnostic interview. Supervision must follow the clinician's license, and treatment must remain within scope.

The separately linked Annual Supervision service definition describes psychologist/LIMHP oversight of a non-independently licensed clinician: treatment-plan and progress-note review, case discussion and annual face-to-face assessment of the Medicaid member, with assistance available in person or by telephone during services. This is a separately defined service; it does not mean every supervision meeting with a trainee is billable, or replace ongoing licensing supervision.

Should the supervisor or PLMHP be the rendering provider?

The staffing definition confirms PLMHP eligibility for the service but does not itself prescribe a universal supervisor-NPI claim convention. The currently linked Nebraska billing instructions, revised May 15, 2024, distinguish the rendering provider in CMS-1500 field 24J for group enrollment from the billing provider in field 33a.

Those general fields do not resolve every provisional clinician's enrollment or supervising-practitioner arrangement. Use the enrolled arrangement and the member's actual payer instructions; do not substitute a supervisor solely because the performer has a provisional license. No universal supervisory modifier is established by the staffing definition reviewed here.

Does this automatically cover every Heritage Health contract?

No. The statewide service definition supplies the permitted staffing category, but it is not proof of a particular Heritage Health plan's network participation, authorization or claim configuration. Keep that plan-specific work separate from the already answered clinical-staffing question.

The Nebraska rules index reports June 28, 2026 amendments to chapters 32 and 35. This article does not restate an older chapter as the current full regulation or treat the proposed replacement manual as adopted. Exact code and setting requirements still govern the actual service.

Frequently Asked Questions

Can a PLMHP provide Nebraska Medicaid individual psychotherapy?

Yes. The currently linked individual-therapy service definition expressly lists PLMHPs, subject to scope, required supervision, medical necessity and the service's other requirements.

Can an unlicensed student use the same rule?

The reviewed staffing list does not establish an unlicensed-student route. PLMHP and provisionally licensed psychologist are specific licensed categories; supervision alone does not turn every student into one.
