Does BridgeSpan Allow Associate and LMSW Supervisory Billing?

Does BridgeSpan allow supervised associate therapy?

Yes, conditionally. BridgeSpan’s Incident To Services policy, Adm148, permits psychological behavioral-health services by qualifying graduate associates and LMSWs under an eligible supervisor. Its specific exception requires same-group supervision, treatment oversight and modifier SA; simply having a supervisor does not satisfy the policy.

Reviewed October 30, 2026. BridgeSpan’s current reimbursement index links the policy reviewed March 1, 2026 and revised effective April 1, 2026. This guide concerns that BridgeSpan policy, not a transfer of Regence, Asuris or Medicare rules. Member benefits, provider agreements and applicable law still control.

Which associates and licenses qualify?

The eligible cohort includes master’s-prepared and doctorate-prepared behavioral-health clinicians who are registered with the appropriate state licensing board and are on a formal licensure track or program. Adm148 also expressly names Licensed Masters Social Workers (LMSWs), subject to state licensing requirements.

The policy does not list every state’s associate-license abbreviation. An Oregon, Idaho, Utah or Washington credential must be assessed against the actual education, board-registration, licensure-track and lawful-practice requirements; the states’ titles are not interchangeable. A pre-degree student or an unregistered graduate is not established as eligible merely by being called an intern.

Who must supervise the services?

The supervisor must be licensed and credentialed, in the same provider group as the associate, and educated to at least the associate’s level. The policy’s non-exhaustive examples include CMHC, LCPC, LCSW, LMFT, LMHC, LPC, PsyD and psychiatrist credentials, subject to state licensing requirements. That regional list does not independently authorize every supervisor–associate pairing in every state.

Services must be part of the member’s normal treatment course. The supervisor must monitor treatment, review the patient’s overall treatment documentation, and be available by telephone or telehealth for consultation as needed. The practice must document meetings with the associate to supervise or monitor care. An outside supervisor from another group does not by itself satisfy the same-group condition.

Associates cannot supervise other associates or provide these incident-to services independently in their own practice. The policy defines the incident-to setting as the practitioner’s office, including an office within an institution, or the patient’s home, and requires the appropriate place of service.

Whose identification and which modifier go on the claim?

Adm148 requires modifier SA on the service line for incident-to consideration and directs submission under the eligible supervisor’s name and identification. It does not provide a complete CMS-1500 or 837P field-by-field mapping of billing, rendering and separate-supervisor identifiers. Do not invent that mapping from the phrase “name and identification.”

Telephone or telehealth availability describes supervision. It does not, by itself, establish coverage for a patient video visit or the required telehealth place of service and modifier combination. Likewise, the initial-service and direct-supervision requirements in the separate palliative-care subsection should not be silently imported into this psychological behavioral-health provision.

Does this include students, residents or fellows?

BridgeSpan’s claims-submission page expressly excludes services rendered as part of a residency or fellowship program and says not to submit them under the attending or supervising provider’s identity. Keep that exclusion alongside the graduate-associate pathway: a formal training placement cannot be assumed covered because a supervisor is available.

For a student still completing the degree, this reviewed policy does not establish eligibility under the graduate cohort. That is a limit of the documented pathway, not a universal statement that every supervised service or every unlicensed person is excluded.

What remains specific to the member and practice?

The reimbursement policy does not define the member’s schedule of benefits or guarantee payment. BridgeSpan’s disclaimer gives governing law, member benefit documents and provider contract terms priority in a conflict. Confirm that the service, clinician arrangement and date of service fit those documents before relying on the incident-to exception.

Frequently Asked Questions

Does BridgeSpan explicitly include LMSWs?

Yes. Adm148 names LMSWs subject to state licensing requirements. Its supervisor, same-group, documentation, service and claim conditions still apply.

Can I bill without SA if the supervisor reviews the notes?

The policy requires SA on the service line for incident-to consideration. Documentation review alone does not replace that requirement or the other supervision conditions.

Does BridgeSpan’s associate rule cover all internships?

No blanket internship permission is established. The cohort requires completed graduate education and qualifying board status, while the claims page separately excludes residency and fellowship services.
