Can a licensing candidate or doctoral student bill under a supervisor?
Yes, conditionally. BCBS Montana’s Provider in Training Policy N006.5, effective March 27, 2026, allows an eligible preceptor to bill for an eligible licensing candidate or doctoral student in a formal supervisor–trainee relationship. The policy applies to BCBSMT lines of business except Medicare Advantage. It is a defined trainee pathway, not permission for every supervised or out-of-network therapist to use another clinician’s NPI.
For counseling, social-work or marriage-and-family-therapy candidates, the operative payer category is an eligible licensing candidate. The policy does not publish a complete credential-abbreviation list or declare every candidate/supervisor pairing lawful. The clinician must actually meet Montana’s applicable licensing and supervision requirements. A doctoral student is expressly named; being any other student, intern or unlicensed employee does not by itself satisfy the policy.

Whose NPI belongs on the claim?
The preceptor submits the claim with their individual rendering NPI. N006.5 says the provider-in-training information is not included on the claim. The NPI submission certifies the preceptor’s personal knowledge that the services meet Montana licensing requirements. Records must reflect oversight, and the member should be informed of the trainee’s status.
When training ends and the clinician obtains licensure, the policy requires the newly licensed clinician to obtain an NPI and submit claims as the rendering provider. Continued employment in the same group does not preserve the trainee exception.
Does BCBSMT’s no-incident-to policy contradict this?
No. Policy N017.2 generally requires the actual treating provider’s NPI and rejects ordinary incident-to billing. It expressly preserves the Provider-in-Training and locum-tenens exceptions. A fully licensed out-of-network therapist awaiting network participation is not automatically a trainee; the May 2026 behavioral-health reminder says non-trainee QHPs should bill under their own license.

Must the supervisor be physically present?
The trainee must have direct, personal supervision, with the preceptor in the immediate vicinity throughout the service to perform or manage it when needed. N006.5 explains that physical presence depends on the Montana statutes and regulations for the profession and service. Remote supervision is permitted under this payer policy when Montana licensing law permits it and all electronic-supervision requirements are met. This is not permission to substitute an occasional retrospective chart review for required availability.
The supervisor and trainee remain responsible for the applicable board’s supervision and recordkeeping requirements. The payer may request supporting records. Coverage, medical necessity and the contracted provider or group’s requirements also remain conditions of reimbursement.
Should trainee claims include HL?
BCBSMT’s May 12, 2026 reminder says HL implementation will occur in the future and instructs providers not to append it until BCBSMT communicates a notification. Our October 30 review did not establish a later activation notice. Do not infer an effective HL date from another Blue plan’s policy; check for a BCBSMT notice applicable to the date of service before changing coding.

What does this answer leave outside the pathway?
Medicare Advantage is expressly excluded from N006.5. The policy is not a Medicare Advantage authorization, a guarantee for every service code, or blanket approval for master’s students who are not eligible licensing candidates. For an eligible trainee, establish the formal relationship and lawful supervision first, then follow the preceptor rendering-NPI instruction and the member’s actual benefit and contract requirements.
