Can PLPCs, LMSWs and PLMFTs Bill Missouri Medicaid?

By Bomi Health • October 30, 2026

Yes, conditionally. MO HealthNet’s current Behavioral Health Services manual permits appropriately licensed PLPCs, LMSWs, PLMFTs and provisional psychologists to provide behavioral-health services for children and adults. They must enroll and practice within license scope. In a group or clinic, the group bills and the individual clinician is reported as performing provider; this is not a general instruction to substitute the supervisor’s NPI.

Sections

Which licenses are recognized?

Section 1.2 names Provisionally Licensed Professional Counselors (PLPCs), Licensed Master Social Workers (LMSWs), Provisionally Licensed Marital and Family Therapists (PLMFTs) and Provisionally Licensed Psychologists (PLPs). The clinician needs a valid applicable license and an accepted participation agreement, with their NPI on file.

The manual explicitly bars these provisional/master’s categories from operating independently, receiving direct payment from MHD or owning their own business in that licensed capacity. LMSWs additionally have a 48-month clinical-practice limit in the manual. Permanent-license updates must be submitted when obtained. Enrollment as a performing clinician therefore does not equal permission to open an independent practice.

Whose NPI and which modifiers apply?

For each of these licensed categories, Section 1.2 says group/clinic services are filed under the group/clinic NPI with the individual’s NPI as performing provider. Clinical supervision remains required where the professional license requires it; it does not make the supervisor the person who performed the service.

Section 1.22 requires the appropriate specialty modifier: AJ for LMSW, UD for PLPC, HE for PLMFT and AH for PLP. Psychology interns have AH and HL. Additional modifiers depend on service and setting; these are not interchangeable with another payer’s associate modifier. ABA has separate coding instructions, including an exception to AH for psychologists.

Do unlicensed student interns qualify?

There is a distinct doctoral psychology-intern exception. The intern must be in good standing in an APA-accredited internship program that is an APPIC member, under direct supervision of the enrolled licensed psychologist responsible for the patient. The supervisor is responsible for work quality, compliance and retaining the intern’s name and training dates.

That exception does not establish general psychotherapy billing for every counseling student or unlicensed master’s graduate. The manual’s general documentation provisions reject services by unenrolled individuals; the specific psychology-intern pathway is an exception, not a blanket delegation rule.

Is this ordinary psychotherapy or only health-behavior assessment?

The general provider-participation rules and psychotherapy sections establish the behavioral-health pathway. Health and Behavior Assessment and Intervention is a separate service directed at factors affecting a physical health condition; its eligibility list is not the sole basis for this answer and it must not be used to treat a behavioral-health condition under the wrong code.

Covered psychotherapy still requires medical necessity, eligible member benefits, appropriate documentation, service limits and precertification when applicable. Managed-care plans have their own program administration; this state manual alone does not establish every plan’s network or contractual approval.

Use the current manual

This review follows the state’s current Behavioral Health Services page to its linked May 2026 Word manual. The older November 2025 and September 2019 files are not used as current authority. The license, rendering and modifier statements above are tied to the current Sections 1.2 and 1.22, not an archived billing example.

Frequently Asked Questions

Should a PLPC’s supervisor be listed as performing provider?

For the licensed PLPC group/clinic pathway, Section 1.2 says to report the individual clinician’s NPI as performing provider under the group/clinic billing NPI.

Does an LMSW’s Medicaid enrollment allow independent practice?

No. The manual restricts independent practice, direct MHD payment and owning a business in the LMSW capacity, and states a 48-month clinical-practice limit.

Can any unlicensed intern bill psychotherapy?

No general student pathway is established. The manual specifies a distinct eligible doctoral psychology internship under an enrolled psychologist.
