Does BCBS Minnesota Allow Trainee Therapy Billing With U7?

By Bomi Health • October 30, 2026

Yes, conditionally. Blue Cross and Blue Shield of Minnesota’s provider manual expressly allows behavioral-health clinical trainees or interns to provide psychotherapy, diagnostic assessments and treatment plans under a licensed behavioral-health professional’s supervision. Its claim rule depends on whether the supervised employee has an individual provider number; it is not a blanket instruction to bill every therapist under a supervisor.

Sections

Who qualifies as a trainee or intern?

The manual names behavioral-health clinical trainees and interns. It also requires practitioners to meet applicable state or federal laws and licensing standards, work within their scope, and follow the Provider Service Agreement and member contract. A degree, student placement or supervisor relationship alone does not establish that a particular person and service qualify.

The cited passage does not enumerate Minnesota license abbreviations such as LGSW or LPC or define every graduate and student category. Match the actual clinician’s legal role and training status to the plan’s requirements rather than treating a license abbreviation as automatic approval. The supervisor must be a licensed behavioral-health professional for this behavioral-health pathway.

When do the supervisor NPI and U7 apply?

Page 8-49 says supervised employees who have not been issued individual provider numbers must have their services submitted under the supervising physician or behavioral-health professional’s NPI, with U7 appended to the HCPCS code. That is a specific identity-and-modifier rule for that employee category.

The same page separately instructs eligible nonphysician professionals to meet Blue Cross credentialing criteria and use their own NPI or individual provider number when submitting claims. Do not remove an enrolled clinician’s identity simply because someone supervises their work. Confirm the clinician’s enrollment and provider-number status before choosing the claim path.

Employment and supervision still matter

The surrounding section addresses employees, including qualifying leased employees, and the employing provider’s responsibility for their work. It does not establish a general independent-contractor exception. Required countersignatures depend on licensure and scope-of-practice requirements.

The following pages separately discuss physician incident-to services and direct versus general physician supervision. Those definitions should not be silently substituted for an independently verified behavioral-health supervision arrangement. The trainee paragraph itself does not specify every permitted supervisor-license pairing or a universal remote-supervision schedule.

Which product does the answer cover?

This answer describes the Blue Cross Minnesota provider manual’s claims-filing rule, subject to the specific agreement and member benefit. It does not certify every commercial, Medicare or Minnesota Health Care Programs product. The payer separates reimbursement policies by line of business and directs Medicaid readers to MHCP guidance when no specific Medicaid policy exists. A Medicaid clinical-trainee rule is not, by itself, proof of commercial coverage.

Frequently Asked Questions

Does every supervised therapist need U7?

No. The cited rule applies to supervised employees who have not been issued individual provider numbers. Eligible professionals with their own numbers have separate claim instructions.

Does being a student automatically qualify?

No. The manual identifies clinical trainees and interns, subject to licensing, scope, employment, agreement and benefit requirements. It does not approve every student placement.
