Does MassHealth Cover Therapy by Unlicensed Graduates and Interns?

Yes, through an eligible mental health center

MassHealth permits eligible mental health centers to bill medically necessary services delivered by specified unlicensed graduates, LCSWs and trainees under supervision. The permission comes from 130 CMR 429.424, not simply from having a supervisor or a master's degree. It applies to the center's covered services and qualified staff; it does not establish an unlicensed clinician's right to enroll or bill independently in private practice.

Reviewed October 30, 2026 against the Mental Health Center Manual, MHC-53, dated September 1, 2025. Community behavioral health centers have a separate regulatory chapter, and managed-care participation and claim instructions must be checked for the member's actual plan.

Which graduate clinicians and students qualify?

Social work: the center rule includes LCSWs with a master's degree and two years of full-time supervised clinical work in an organized graduate internship; unlicensed graduates with a CSWE-accredited master's in social work; and second-year clinical-track MSW students in a structured practicum in a fully CSWE-accredited program. These are separate categories, not a blanket permission for every social-work student.

Counseling: post-master's mental health counselors need a master's degree or higher in a mental health field from an accredited institution plus one year of supervised clinical work in an organized graduate internship. Counselor interns need a second-year clinical-track structured placement in a counseling or counseling-psychology master's program accepted by the Massachusetts board.

Marriage and family therapy: the rule similarly includes qualifying post-master's clinicians with the required graduate internship and second-year clinical-track interns in a board-accepted marriage/family therapy or related master's program.

Psychology: qualifying postdoctoral fellows must meet the doctoral-degree and board experience/supervision requirements. Psychology interns must be enrolled in the specified structured APA-approved clinical or counseling doctoral program. A general undergraduate placement is not this category.

What does direct and continuous supervision require?

The manual defines it as ongoing supervision of at least one hour weekly for full-time employees, prorated for part-time employees. Its definition requires an independently licensed staff member or certified peer supervisor employed by the agency; the peer-supervisor category does not independently authorize supervision of psychotherapy outside applicable professional scope.

Section 429.423 permits telehealth supervision. The supervising clinician remains primarily responsible for care, chart review must be documented, and supervision records must include frequency, format, the supervisor's signature and credentials, and the general content discussed. Clinicians delivering individual, group or family therapy to members under 21 also need the required CANS certification every two years.

Does the claim use HL, HO or the supervisor's NPI?

MHC-53's service-code section requires a licensure-level modifier for listed psychotherapy codes, including 90834 and 90837. Its HL definition names intern-level categories including postdoctoral fellows, psychology interns, post-master's counselors, counselor interns and post-master's marriage/family therapists. HO names master's-level clinicians including LCSWs and LICSWs. Use the category matching the actual performer and service; a master's degree alone does not resolve every trainee's modifier.

The modifier table is not a separate staffing permission. For example, an entry in that table cannot override 429.424's limits on direct services. These sources establish center billing and professional qualifications; they do not settle every trainee's rendering-NPI field. Do not substitute a supervisor's NPI merely because the staff member is supervised. BHUC encounter bundles also have their own instructions.

Can an independent therapist use this center permission?

An independent office cannot assume the mental health center enrollment, staffing and service rules apply to it. Confirm the enrolled provider type and service location first. For a center, retain proof of the trainee's precise education/placement category and supervision before submitting the center's covered claim. A valid intern placement establishes neither every managed-care contract nor payment for every service.

Frequently Asked Questions

Can MassHealth cover therapy by an unlicensed MSW graduate?

Yes through an eligible mental health center when the graduate meets 429.424's CSWE-accredited MSW qualification and the center meets supervision, medical necessity and other service requirements. This is not independent private-practice enrollment.

Does every supervised student qualify?

No blanket student permission is established. The regulation specifies particular psychology programs and second-year clinical-track social work, counseling and marriage/family placements.
