Does CCA Allow Supervised Intern Therapy in Massachusetts?

Does CCA cover therapy provided by interns or postmaster clinicians?

Yes, conditionally for the Massachusetts Senior Care Options and One Care products covered by CCA’s 2026 Provider Manual. Its licensure table permits specified postmaster clinicians and master’s-level interns to provide and bill direct services under an independently licensed clinician’s direct personal supervision at a mental health center, community health center or community behavioral health center. The table assigns HL to these categories. This is a center-based supervised-care pathway, not blanket permission for every unlicensed worker or private practice.

Which training categories does the manual name?

The postmaster row names social work, counseling and clinical psychology, with MSW, MA and MS credentials. A separate row names master’s-level interns, including psychology interns, mental health counselors and marriage and family interns. Both rows carry the same direct personal supervision and specified-center conditions. A student label alone is insufficient: match the person’s training category and the center’s requirements rather than treating all undergraduates, trainees or unlicensed graduates alike.

Who must supervise, and where can the services occur?

For those two rows, CCA requires an independently licensed clinician and direct personal supervision at one of the three named center types. A general outpatient benefit that also covers a practitioner’s office does not erase this narrower setting restriction. The source does not define these rows as an unrestricted home-office, solo-practice or ordinary group-practice exception. The clinician and organization must also satisfy applicable Massachusetts scope, supervision, participation and enrollment requirements.

Does HL mean the supervisor’s NPI goes on the claim?

No such conclusion follows from HL alone. CCA’s outpatient payment policy identifies HL with intern-level services and names categories including psychology interns, postdoctoral fellows, postmaster mental health counselors, counselor interns and postmaster marriage and family therapists. Its general professional-claim instructions identify a rendering-NPI field, but the reviewed passages do not provide a complete trainee-versus-supervisor NPI crosswalk for this center-based pathway. Confirm the rendering and billing identities for the actual enrolled center and service before submitting; do not substitute a supervisor solely because HL appears on the claim.

What other payment conditions apply?

The outpatient policy covers medically necessary behavioral-health services and lists individual psychotherapy codes 90832, 90834 and 90837. It requires Medicare and/or MassHealth enrollment depending on licensure. It generally allows one visit of a single service type per member per day, excluding diagnostic services, and applies service limits and coding edits. In-network outpatient behavioral-health services generally do not need prior authorization under this policy, with named exceptions; out-of-network services require it. Eligibility, benefits, the center’s contract and the actual service still govern payment.

Which CCA plans and policy dates does this answer cover?

This answer is limited to Massachusetts CCA Senior Care Options and One Care, identified as FIDE-SNP products in the 2026 manual and outpatient policy. The current manual index labels the reviewed edition updated June 1, 2026. The outpatient policy shows an effective date of December 1, 2025 and revision date of July 1, 2026. These dates do not establish a new start date for every trainee category. The sources were reviewed October 30, 2026. They do not establish the same permission for CCA products in other states or a separate Medicare Advantage product.
