Does Anthem Kentucky Allow LPCA or CSW Supervisory Billing?

Does Anthem’s incident-to policy authorize an LPCA or CSW?

No—not on the basis of commercial policy C-11002 alone. The version effective December 2, 2025 defines its non-physician practitioner category as nurse practitioners and physician assistants. It does not establish supervisory-billing eligibility for licensed professional counselor associates, certified social workers, marriage-and-family associates or student therapists.

This is a limit on what this policy proves, not a finding that every Anthem Kentucky product denies supervised therapy. A separate behavioral-health contract or product policy may govern. The reviewed commercial document cannot supply that missing permission.

What does Kentucky’s exception actually change?

The policy normally applies a 15% reduction to the supervising physician’s maximum allowance for qualifying incident-to services. Its Kentucky exemption removes that payment reduction. It does not add therapy associates to the NPP definition or remove the policy’s supervision requirements.

The same table expressly exempts Colorado from the SA modifier; it does not give Kentucky that modifier exemption. For the NP/PA services covered by this policy, SA remains the stated non-surgical incident-to modifier. That is not an instruction to put SA on an LPCA or CSW psychotherapy claim.

Who renders and who supervises under this policy?

A physician or eligible NPP recognized to submit claims directly must report services under their own NPI. The policy does not permit a directly eligible NPP to use incident-to billing merely while waiting for an NPI or health-plan eligibility. For an otherwise qualifying NPP who cannot submit directly, it describes billing under the supervising physician’s NPI.

The physician must be physically present and/or immediately available as the policy specifies, remain actively involved in care and document review of each service. These physician/NP/PA provisions are not interchangeable with clinical supervision by a therapist.

Are a supervised associate and a student the same category?

No. Kentucky’s counseling definitions distinguish an LPCA working with clients under supervision from a student enrolled in counselor education. State authority to practice under supervision does not by itself establish Anthem reimbursement. Neither category appears in C-11002’s NP/PA definition.

Which products remain unresolved?

This review concerns Anthem Kentucky commercial incident-to policy. It does not establish Anthem Medicaid, Medicare Advantage, a delegated behavioral-health arrangement or a particular employer’s coverage. An LPCA, CSW or MFT associate claim needs an applicable behavioral-health source identifying the credential and claim arrangement; the commercial NP/PA exception is not that source.

Frequently Asked Questions

Does Kentucky’s incident-to exception mean all associate therapy is covered?

No. The exemption removes a payment reduction within C-11002. Its NPP definition lists NP and PA, not therapist associates.

Can SA make an LPCA psychotherapy claim eligible?

No. A modifier cannot create payer permission. C-11002’s SA instruction belongs to qualifying NP/PA incident-to services.
