Does Indiana CSHCS pay for supervised mental-health therapy?
Ordinarily, no. Indiana Children’s Special Health Care Services (CSHCS) lists services for mental-health conditions, counseling, testing and substance-abuse treatment among its coverage exclusions. Its separate exclusions policy also lists psychiatric treatment and mental-health services. This is a benefit restriction, so using a licensed supervisor’s credentials does not by itself make the service covered. Policy C-4 allows exceptions approved by designated program officials; that approval must be established individually, not assumed from supervision.
That answer is specific to CSHCS. It does not prohibit an Indiana associate or student from practicing under lawful supervision, and it does not decide coverage under Indiana Medicaid or a family’s private insurance.
Does the answer change for an LMHCA, LMFTA, LSW or student?
The cited exclusion concerns the mental-health service, rather than an individual license category. It does not establish a routine psychotherapy benefit for an independently licensed therapist either. A practice should not treat an LMHCA, LMFTA, LSW or student as eligible for an excluded service merely because a supervisor signs the note or appears on the claim.
These sources do not provide a supervisee-to-supervisor license pairing, a rendering-NPI substitution rule or a supervision modifier that overrides the exclusion. No such claim recipe is established here.
Can CSHCS approve an exception?
Policy C-4 reserves exceptions requiring approval by the Best Practices Committee, the Medical Consultant (physician) or the Program Director. The ordinary exclusion therefore should not be described as an absolute rule with no exceptions.
An exception is an individual program decision, not an automatic allowance for supervised therapy. A referral, a supervisor’s signature or a provider’s enrollment does not establish that this approval has been granted. Any approved service still needs the applicable participant, provider, authorization and claim requirements.
Why do Medicaid rates and other therapy services appear in the manual?
CSHCS is a supplemental program and payer of last resort for authorized services related to eligible medical conditions. The manual expressly says it is not Medicaid, even though it uses Indiana Medicaid rates, coding and claim forms. Ordinary Medicaid therapist rules cannot be imported on that basis.
The program’s authorization materials separately list occupational therapy, physical therapy, speech therapy and ABA. Those service categories do not create a general mental-health counseling benefit. A practice must identify the actual service and program benefit rather than relabel psychotherapy as another kind of therapy.
Which authorization instructions are current?
The current prior-authorization page announces that, effective January 1, 2026, CSHCS no longer has linked providers and prior authorization is required for all services. Older linked-provider wording remains below that notice and in the November 2022 manual; it should not be used as a current authorization shortcut.
Prior authorization is not a payment guarantee. Participant eligibility, other coverage and the program’s claim requirements still matter. The authorization update does not itself remove the mental-health exclusion or grant a therapist-license exception.
Sources and review date
Reviewed October 30, 2026. The current state resources page links the November 2022 provider manual, and the policy library links C-4, last revised January 1, 2023. The exclusion is on printed page 17 of the manual and page 3 of C-4; the exception authority is on page 1 of C-4. The separate 2026 authorization notice supplies the newer operational limit.
