Does Delaware DSCYF Allow Unlicensed Graduate Therapists?
By Bomi Health • October 30, 2026
Yes, within a defined agency pathway. Delaware’s Division of Prevention and Behavioral Health Services (DPBHS), part of DSCYF, recognizes unlicensed graduates working toward professional licensure in its contracted children’s outpatient mental-health program. Degree qualifications, agency approval, supervision and staff registration all apply. This is not permission for every private practice or Medicaid plan to bill a graduate’s therapy under a supervisor’s NPI.

Which graduates qualify for the mental-health pathway?
The currently linked May 2023 provider manual, printed page 133, requires at least a master’s or doctoral degree in psychology, social work or another human-service field that qualifies the person for Delaware licensure as an LCSW, LPCMH, LMFT or psychologist. Unlicensed clinicians are expected to be obtaining the supervision hours needed for licensure. These are the licenses the education must qualify toward; the passage does not require the graduate already to hold one of those independent licenses.
A completed qualifying degree is different from being a current student. This provision does not establish eligibility for a student who has not completed that degree, or for every person described simply as unlicensed.
Must the therapist work through an agency?
Yes. Printed page 9 requires an unlicensed practitioner providing Medicaid children’s behavioral-health services to operate within an agency meeting DSCYF requirements. The current provider page describes contracting and requires DPBHS contracted providers to enroll with Delaware Medicaid before billing. Applicable professional, organizational, contract and service requirements remain in force.
The mental-health outpatient section covers individual, family and group therapy in office, home and community settings. Those settings do not remove the agency requirement. The child’s program eligibility, clinical necessity, admission and authorization requirements also matter; this is a children’s program, not a general adult private-practice benefit.
What supervision and signatures are required?
Printed pages 12 and 133 require weekly supervision by a licensed mental-health professional. The supervision log records the staff member, date, duration and number of cases reviewed. The licensed supervisor signs the log, assumes clinical responsibility and signs assessments, treatment plans and discharge summaries.
The manual separately addresses substance-use outpatient services and CADC/LCDP qualifications. Those provisions are not an alternative supervisor qualification for the mental-health pathway described here.
What must be registered before reimbursement?
Printed page 11 requires the human-resources form and an assigned Staff Identification Number for each behavioral-health professional providing services and submitting claims. DPBHS states that it will not reimburse services delivered by a professional without that assigned number. Agency approval and a supervisor’s signature do not replace this staff-registration requirement.
The Staff Identification Number is not a rendering-NPI instruction. These provisions do not establish a supervisor-NPI substitution, an associate-specific modifier or a universal claim recipe. Keep the treating clinician’s qualifications and registration separate from the claim’s billing and rendering fields and the applicable contract.
Frequently Asked Questions
Does DSCYF recognize unlicensed graduates for outpatient mental-health therapy?
Yes, within its contracted children’s program: a qualifying completed graduate degree, an approved agency, staff registration, weekly licensed mental-health supervision and the other service and contract requirements apply. This is not an unrestricted private-practice billing permission.
Does this establish a pathway for students without a completed graduate degree?
No such exception is established by the reviewed mental-health outpatient provision. Its qualifying master’s or doctoral degree requirement must not be replaced with generic student or unlicensed status.
Does the supervisor’s signature determine which NPI goes on the claim?
No. The manual requires clinical supervision, signatures and an assigned staff ID, but these passages do not establish a supervisor rendering-NPI substitution or modifier.
Sources and currentness
Reviewed October 30, 2026. The current DSCYF provider page links and incorporates the treatment-provider manual revised May 2023. This guide uses printed pages 8–12 for agency, registration and supervision requirements and pages 132–135 for mental-health outpatient services. The current link establishes adoption of that dated manual; it does not make every older administrative allocation a newly effective 2026 rule.
