Does Medi-Cal Cover AMFT, ASW, APCC and Student Therapy?

Can AMFTs, ASWs and APCCs provide Medi-Cal therapy?
Yes, conditionally. The October 2026 non-specialty mental-health manual names associate marriage and family therapists, associate clinical social workers, associate professional clinical counselors and registered psychological associates. They may provide mental-health services within their qualified supervisor’s scope, under the applicable licensing-board rules.
Separate non-specialty mental-health services (NSMHS) from county specialty mental-health services (SMHS). The state manuals describe different claiming systems; neither a supervisor’s license nor an associate registration by itself establishes an agreement with every managed-care plan or county.

Non-specialty services: who bills and signs the notes?
The October 2026 billing manual makes the qualified supervising licensed clinician the billable provider and requires the supervisor to sign, review and approve the associate’s clinical documentation, including assessments and treatment recommendations. Required oversight agreements, approved supervision and supervision hours must also be maintained.
For CMS-1500 claims, identify the associate by name in Box 19 or an attachment and use the supervisor’s NPI as billing provider. The institutional instruction instead identifies the associate practitioner type in UB-04 Box 80. These billing instructions do not establish a universal associate rendering-NPI substitution or an HL requirement for every NSMHS claim. FQHC/RHC encounter rules and managed-care submission arrangements remain separate.

County specialty services: registrants and clinical trainees differ
The July 2026 SMHS manual distinguishes registered pre-licensed professionals from clinical trainees. For registrants it specifies HL and allows their own rendering NPI when available; otherwise it identifies the licensed supervisor’s NPI and taxonomy as rendering. It identifies the supervising professional as billing provider.
Clinical trainees use a different protocol. Section 5.17 lists taxonomy beginning 3902 with AJ for LCSW/MFT/LPCC clinical trainees, or AH for psychologist clinical trainees. Rendering and supervising identities are reported separately; the required supervisor is the licensed clinician who co-signed the progress note. Do not apply the registrant HL recipe indiscriminately to clinical trainees. County service tables, allowed disciplines and supervision qualifications still control.

Can a student or a graduate awaiting registration qualify?
BHIN 24-023 permits defined clinical trainees in county behavioral-health programs. A trainee must be in the relevant educational program and an approved practicum, clerkship or internship, meet coursework and supervised-practice requirements, and provide services within lawful scope. Merely being a student is insufficient.
The notice also allows qualifying master’s graduates pursuing CSW, MFT or PCC registration to provide specified services while a timely BBS associate application is pending. The application must have been submitted within 90 days of the degree award; the delivery system must retain proof and later verify approval. If BBS does not approve it, the services are not reimbursable. This county-program provision is not an automatic exception for every NSMHS private-practice claim.
Which answer applies to your next client?
Match the clinician’s actual status—registered associate, qualifying pending applicant or clinical trainee—to the service and delivery system. Then establish the responsible plan or county’s participation and submission requirements. A missing product-specific answer remains unresolved; it does not cancel the defined pathways above or create permission outside them.

Frequently Asked Questions
Does every student qualify for Medi-Cal psychotherapy reimbursement?
No. County clinical trainees must meet the defined education, placement, supervised-practice and service requirements. Associate eligibility is a separate status.
Should every supervised Medi-Cal service use HL?
No. The county SMHS manual distinguishes registrants using HL from clinical trainees with profession-specific coding. NSMHS has its own billing instructions.
