Does CenCal Pay for AMFT, ASW and APCC Therapy?
By Bomi Health • October 30, 2026
Yes. CenCal Health publishes a Medi-Cal mental-health claims pathway for AMFTs, associate clinical social workers (called ACSWs in its guide), APCCs and psychological assistants working under qualified licensed supervision. Its mental-health claims guide identifies these associate services with HO and instructs providers to use the fully licensed supervising clinician’s NPI in the rendering field. This is a named associate pathway, not permission for any student or unlicensed employee to provide reimbursable psychotherapy.

What services and product does this cover?
The 2026 CenCal provider manual, section E12, covers non-specialty mental-health services, including individual, family and group psychotherapy for eligible Medi-Cal members. It lists individual psychotherapy codes 90832, 90834 and 90837. The service must fit the member’s covered benefit and the clinician’s lawful scope; network participation and applicable medical-necessity, documentation and authorization requirements still matter.
Keep this separate from county specialty mental-health services, ABA and Medicare CareConnect. A CenCal Medi-Cal associate claim instruction does not establish the same rule for those programs or for another Medi-Cal managed-care plan.
Which NPI goes where?
The Mental Health Claims Entry guide’s rendering-NPI instruction names AMFT, ACSW, APCC and Psych Assistant claims and calls for the fully licensed supervising clinician’s NPI. For its portal workflow, it says to leave the rendering field blank if that supervisor’s NPI is already the billing NPI. That blank-field instruction is specific to the described entry workflow; it is not an instruction to omit a required provider loop from an electronic 837P claim.
The guide also permits the associate’s NPI in Reserved for Local Use / Remarks. Do not replace the supervisor rendering instruction with the associate’s own rendering NPI solely because the associate has an NPI. The practice still needs the correct contracted billing identity and approved supervisor arrangement.
Is HO an associate-therapy modifier here?
Yes—the mental-health claims guide explicitly describes HO for services performed by an associate psychotherapist (AMFT, ACSW or APCC) or psychological assistant under licensed supervision. CenCal’s currently linked LMFT orientation also says HO must be used with the named supervised categories. This conclusion comes from mental-health claim instructions, not from interpreting an ABA staffing or education modifier table.
What must the practice establish before billing?
The 2026 manual’s credentialing section requires a supervisory letter for associate behavioral-health providers. That onboarding document alone is not the payment rule: the specific claim instructions above supply the additional evidence. Confirm the clinician’s registration or applicable authorization, lawful supervisor qualifications, the practice’s CenCal participation and the service’s requirements. An ordinary student placement is not automatically one of the named categories.
How current are these instructions?
Reviewed October 30, 2026: CenCal’s provider-manual index lists its 2026 manual, and its Welcome to the Network page still links the LMFT orientation dated 2023. The Mental Health Claims Entry guide carries identifier P-Claims-MHUG-E-0922 and is hosted in the payer’s February 2025 document directory. Those older documents supply the specific associate NPI and HO instructions; the 2026 manual supplies current benefit and onboarding context. This guide does not treat every older training statement, particularly Medicare coordination guidance, as newly validated.

Frequently Asked Questions
Can any student bill CenCal under a supervisor?
The reviewed instructions name specific associate and psychological-assistant categories. They do not establish a general student or unlicensed-employee pathway.
Does CenCal use the associate’s own rendering NPI?
The reviewed mental-health guide instructs the practice to use the fully licensed supervising clinician’s rendering NPI. Its portal leaves rendering blank if that NPI is already the billing NPI; this does not define an electronic 837P omission rule.
