Can an associate therapist bill the AHF-administered San Diego Ryan White Specialty Services Program?
Not for ordinary mental or behavioral health treatment under this specialty-services benefit. The program’s currently linked provider manual expressly lists mental/behavioral health services among its exclusions. Using a licensed supervisor’s NPI or adding a supervision modifier would not make an excluded service covered.
This is a specific San Diego County program administered by AIDS Healthcare Foundation (AHF), not a rule for every AHF clinic, insurance product or Ryan White service. The manual identifies payer ID 95433 for Ryan White programs. That identifier does not establish that every claim bearing the broader AHF name has this benefit design.
Is the restriction about the therapist’s license?
The cited restriction is about the service benefit, not a list of disallowed associate licenses. It does not create a different outpatient psychotherapy entitlement for an AMFT, ASW, APCC, psychological associate, student or independently licensed therapist. The reviewed specialty-program manual does not supply a supervisor-billing exception to its mental/behavioral-health exclusion.
This does not mean those clinicians can never provide covered services through another San Diego County program. Their eligibility in a different benefit requires that program’s provider qualifications, supervision and service rules.
Why does the manual also mention counseling?
Keep the named benefit categories separate. The manual includes medical social work within professional home-health care, describing crisis intervention, planning, counseling, resource information and referrals. It also describes counseling within hospice services. Those references are not a general outpatient psychotherapy benefit and do not erase the separate mental/behavioral-health exclusion.
The specialty benefit covers authorized services for eligible clients when the condition is related to HIV/AIDS. A practice should assess the actual service category, authorization and program requirements rather than relabeling psychotherapy as home-health or hospice counseling.
Where does mental health care fit in the wider Ryan White program?
The county’s current Ryan White provider-resources page separately lists psychiatric and mental health services in its enrollment resources. It also links both a general 2026 Provider Handbook and the 2023 Specialty Services Program manual. The existence of that wider mental-health program does not place mental-health visits inside the AHF-administered specialty benefit.
For a psychotherapy referral, determine which county benefit or other coverage is responsible and then establish the therapist’s eligibility under that program. This article does not identify an alternative payer or guarantee that an associate, student or supervisor can bill it.
Which source and dates support this answer?
Reviewed October 30, 2026. The county’s current provider-resources index, updated July 20, 2026, still links the Specialty Services Program manual revised December 2023. That current adoption supports using the manual for this bounded program answer; the older publication date alone does not mean it was withdrawn.
The exclusion appears on printed page 6. The administrator and program description appear near the beginning of the manual, and the claims section identifies 95433. This article does not establish an inception date for the exclusion, resolve earlier dates of service, or extend it to AHF’s other products.
