Can an LAC or LMSW provide AHCCCS counseling?
Yes, through the applicable supervised, licensed-facility pathway—not simply by billing under an independently licensed supervisor’s NPI. AHCCCS’s Covered Behavioral Health Services Guide recognizes associate-level behavioral health professionals (BHPs) as non-independent billers, subject to Arizona Board of Behavioral Health Examiners supervision requirements. Its official FAQ identifies LMSW, LAC and LASAC as examples of associate-level BHPs.
The current guide distinguishes independently licensed clinicians using CPT codes for services they personally perform from eligible associate BHPs and behavioral health technicians (BHTs) using the specified facility/HCPCS pathway. An associate license alone does not authorize independent private-practice billing.

Whose NPI is used for associate counseling?
For the specified non-independent-biller services, use the licensed behavioral health facility’s NPI as servicing/rendering provider and identify the person who actually furnished the service according to the health plan or fee-for-service instructions. Core Billing Limitation 18 provides this rule; the guide’s provider section describes reporting the individual in the claim narrative, including 837P loop 2300 NTE.
The counseling section expressly prohibits outpatient incident-to billing under the NPI of a BHP who did not perform the counseling. Supervisory review does not turn the supervisor into the treating clinician. A group-payment identifier is also not interchangeable with a licensed facility’s rendering identity.
What service and supervision rules apply to H0004?
The guide identifies H0004 for behavioral health counseling and therapy, per 15 minutes. It requires an associate licensed BHP engaged in direct supervision under AAC R4-6-211, or a BHT with BHP clinical oversight under the applicable health-institution requirements. For this BHT pathway, the technician must work for an ADHS-licensed behavioral health facility.
Group counseling for two or more members uses HQ with H0004. The guide also describes HR/HS family-counseling distinctions, service-plan documentation and restrictions on billing alongside per-diem programs. These modifiers describe the service; none is a universal supervision modifier. Apply the actual setting, service and same-day rules rather than billing every visit as H0004.
Does BHT mean any unlicensed counselor or student?
No. A BHT is a defined staff category working at or for a licensed health care institution under the applicable education, experience, oversight and facility requirements. It is not a synonym for anyone hired without an independent license. A student may not be treated as an eligible BHT merely because an independently licensed clinician signs the note.
Associate BHPs, BHTs and behavioral health paraprofessionals are distinct categories. The counseling pathway does not establish blanket psychotherapy permission for paraprofessionals, all students or every rehabilitation worker. IHS/638 and tribal programs have separately referenced billing requirements and should not inherit a nontribal facility claim recipe.
Does this apply to managed care as well as fee-for-service?
AMPM 310-B covers the listed ACC, ACC-RBHA, ALTCS E/PD, DCS CHP and DES DDD contractors and specified fee-for-service programs, excluding the Federal Emergency Services Program. That broad benefit scope does not remove plan-specific contracting, authorization, member coverage or submission requirements. The guide itself directs claim-name reporting to the applicable health-plan or fee-for-service instructions.

Where is the associate-license clarification?
The official FAQ explains that associate-level BHPs are not independent billers and that clinical oversight/review is not separately billable. Read those clarifications with the current service guide and the actual Arizona supervision rules.

Frequently Asked Questions
Can an LPC bill AHCCCS under their NPI for an LAC’s counseling?
The reviewed outpatient counseling rule prohibits using a BHP’s NPI when that BHP did not perform the counseling. Eligible associate services follow the licensed-facility/HCPCS pathway instead.
Can any unlicensed employee be billed as a BHT?
No. BHT qualifications, facility requirements, clinical oversight and the allowed service must all be satisfied.
