TheraNest vs Jane: Costs and Fit for Therapists
By Bomi Team • October 14, 2026
Comparing TheraNest and Jane? Start with the practice you actually run: the people who need access, the sessions you see, and who handles insurance work. This guide separates subscription costs from AI and billing, then explains when Bomi belongs on the shortlist.
Disclosure: this comparison is published by Bomi. Prices and feature statements are linked to vendor sources. We have not run a comparative product-quality benchmark, and a cheaper subscription is not proof of a better clinical workflow.

Insurance-first and clinic-wide needs can point in different directions
Test the insurance workflow if it is central to the practice, and test broader scheduling requirements if you operate a multidisciplinary clinic. Plan names alone cannot resolve that choice. Ask for both a software estimate and a list of tasks that remain manual.
How we compare costs at 20 sessions a week
Our scenario is one clinician, individual telehealth, secure client messaging, and one scheduler or biller user. We model 20 completed sessions weekly: 20 × 52 ÷ 12 = 86.67 monthly on average, rounded to 87 for transaction examples. Time off, cancellations, and your actual usage can change the bill.
Prices were checked October 14, 2026. We use standard US monthly rates after promotions, with optional AI for one note per session. Annual license fees are listed separately. These are selected configurations, not a complete feature-equivalence audit or the cheapest possible plan for every clinician.
EHR / selected plan | Software monthly, before AI | With AI on 87 sessions | Annual license fee |
|---|---|---|---|
TheraNest — Advanced | $59.00 | $94.00 | $19.50 |
Jane — Practice | $79.00 | $94.00 | $0.00 |
Software totals exclude claim and eligibility transactions, card processing, taxes, paid reminders, other third-party charges, and human billing services. Jane’s optional insurance tools are also excluded. Bomi’s required billing fee is additional in every Bomi total; it cannot be omitted from a complete purchasing estimate.
TheraNest: Advanced supplies the modeled telehealth and messaging features. The $19.50 annual CPT license fee is separate; usage above included allowances is excluded.
Jane: Practice avoids Balance’s 20-appointment monthly cap. Insurance Billing, its clearinghouse account, and group telehealth are excluded from the displayed software total.
What to know about TheraNest
TheraNest is worth comparing when the choice turns on insurance features and plan allowances. Its Essentials, Advanced, and Premier tiers have materially different inclusion rules, so the lowest subscription is not a stand-in for every practice.
Essentials is $29, Advanced $59, and Premier $89 per therapist monthly. Advanced includes telehealth and secure messaging and lists monthly practice-level allowances for claims, eligibility checks, and text or phone reminders. Premier advertises unlimited versions subject to its usage policy.
Ensai notes enhancement included in a plan is distinct from the paid Session Assistant scribe. This comparison uses the $35 per-provider Session Assistant add-on, not a claim that all AI costs extra.

What to know about Jane
Jane is worth evaluating when you want to compare clinic scheduling and documentation alongside therapy-specific platforms. For this caseload, its Practice plan is the useful baseline rather than its limited-volume Balance plan.
Practice is $79 monthly and includes unlimited appointments, staff profiles, one-to-one telehealth, and secure messaging. Balance is $59 but limited to 20 appointments per month, not 20 per week. Insurance Billing is a separate $20 monthly add-on for the first practitioner; the clearinghouse account is not included.
Jane lists five free AI notes per month and a $15 monthly unlimited AI Scribe subscription per opted-in practitioner. Check recording and dictation workflows against your documentation needs.

Which should you shortlist?
TheraNest: You want to compare plan allowances with your insurance volume and may benefit from the difference between Advanced and Premier.
Jane: You want to evaluate a broader clinic platform and can separate cash-pay needs from optional insurance tools.
Model the practice-wide allowance with your actual clinician count. A shared allowance can work differently for a solo practice and a group, even when the per-clinician subscription looks predictable.
Use the trial or demo to test the intake and charting workflow you need for therapy. If insurance matters, examine the Insurance Billing add-on and its clearinghouse costs separately from a cash-pay workflow.
Bomi as a third option
Bomi makes sense to investigate if you also want an in-house team handling billing. Its EHR is $29 monthly per clinician plus required billing at 4% of net collections. AI notes cost $1 per recorded telehealth session. At 87 sessions with AI on every visit, software and AI are $116 before the required billing fee and third-party charges.
Bomi is not VC-backed, and our billing team is in-house. That describes how we operate; it does not establish how another vendor staffs its services. Ask every provider who performs benefits checks, payer follow-up, and denial work, and how the practice can review that work.
For a self-billing practice, managed billing adds a new expense. For a practice already paying a biller, compare the full package with that existing fee. At an illustrative $10,000 in monthly net collections, Bomi’s required 4% fee is $400; the modeled EHR and all-session AI bring the subtotal to $516 before third-party charges.

Switching from TheraNest to Jane
Before switching from TheraNest, review exports for clinical documents, attachments, and billing history. Set responsibility for claims already submitted and verify that the new biller can distinguish an unpaid claim from a historical balance.
Before canceling, ask the destination for an import checklist and a sample reconciliation. Verify client count, signed notes, attachments, active treatment plans, future appointments, payer details, and open balances. Keep a written cutover date and an owner for old claims. Do not assume payer enrollments, payment credentials, or every document format move automatically.
Run a complete workflow with a synthetic client: intake, scheduling, documentation, billing, payment review, and export. Record missing fields and manual steps. A successful demo is more useful than a long feature checklist, especially when you depend on custom forms or staff permissions.
