SimplePractice vs TherapyAppointment: Costs and Fit for Therapists
By Bomi Team • October 14, 2026
Comparing SimplePractice and TherapyAppointment? Start with the practice you actually run: the people who need access, the sessions you see, and who handles insurance work. This guide separates subscription costs from AI and billing, then explains when Bomi belongs on the shortlist.
Disclosure: this comparison is published by Bomi. Prices and feature statements are linked to vendor sources. We have not run a comparative product-quality benchmark, and a cheaper subscription is not proof of a better clinical workflow.

Monthly AI and per-note AI reward different usage
A clinician using AI occasionally should calculate paid notes rather than assume an unlimited subscription saves money. A clinician documenting every appointment should do the opposite. Check how each tool handles edits and session inputs before treating the generated notes as interchangeable.
How we compare costs at 20 sessions a week
Our scenario is one clinician, individual telehealth, secure client messaging, and one scheduler or biller user. We model 20 completed sessions weekly: 20 × 52 ÷ 12 = 86.67 monthly on average, rounded to 87 for transaction examples. Time off, cancellations, and your actual usage can change the bill.
Prices were checked October 14, 2026. We use standard US monthly rates after promotions, with optional AI for one note per session. Annual license fees are listed separately. These are selected configurations, not a complete feature-equivalence audit or the cheapest possible plan for every clinician.
EHR / selected plan | Software monthly, before AI | With AI on 87 sessions | Annual license fee |
|---|---|---|---|
SimplePractice — Plus | $99.00 | $134.00 | $20.00 |
TherapyAppointment — Standard solo + telehealth | $74.00 | $125.33 | $0.00 |
Software totals exclude claim and eligibility transactions, card processing, taxes, paid reminders, other third-party charges, and human billing services. Jane’s optional insurance tools are also excluded. Bomi’s required billing fee is additional in every Bomi total; it cannot be omitted from a complete purchasing estimate.
SimplePractice: Plus is selected for scheduler or biller access; it is not the lowest solo plan. A $20 annual AMA fee is shown separately. Insurance transactions are additional.
TherapyAppointment: The total is $59 standard subscription plus $15 integrated telehealth. Lower introductory tiers are excluded. Electronic claims, eligibility, and card processing are additional.
What to know about SimplePractice
SimplePractice is worth considering if you want a tiered mental-health practice platform and expect to add staff or clinicians. The plan decision matters: a solo clinician without staff access may need less than the Plus configuration used here.
Starter, Essential, and Plus have different feature sets. Essential may be sufficient when you need telehealth and secure messaging but do not need the staff access modeled here. Compare the recurring plan rate after the introductory offer, not the discounted first invoice.
Note Taker is priced separately from the broader Care Aide suite. The comparison uses the standard standalone Note Taker rate, not a promotional price or the price of the entire suite.

What to know about TherapyAppointment
TherapyAppointment is useful to evaluate when you want free administrative accounts and usage-based AI notes. Its low-volume introductory subscription should be separated from the standard rate for an established caseload.
At 40 or more booked sessions in a month, the published standard subscription is $59. Integrated telehealth adds $15 per provider. Administrative accounts and secure messaging are included. The lower introductory tiers do not represent this 87-session month.
AI Notes are $0.59 per chart note with unlimited free edits. A generated chart note and a recorded session are different billing units; the model assumes one paid note for each completed session.

Which should you shortlist?
SimplePractice: You want to evaluate a mental-health platform with several plan tiers and can match the subscription to your staffing needs.
TherapyAppointment: You want included administrative access and prefer to pay for generated AI chart notes as you use them.
Test how your scheduler can manage appointments without unnecessary clinical-record access. If you run groups, also test the group appointment and telehealth workflow on the plan you intend to purchase.
Test the claim progress logs and problem-claim notifications with the person who will act on them. Check whether your existing telehealth setup changes which optional video integration you would purchase.
Bomi as a third option
Bomi makes sense to investigate if you also want an in-house team handling billing. Its EHR is $29 monthly per clinician plus required billing at 4% of net collections. AI notes cost $1 per recorded telehealth session. At 87 sessions with AI on every visit, software and AI are $116 before the required billing fee and third-party charges.
Bomi is not VC-backed, and our billing team is in-house. That describes how we operate; it does not establish how another vendor staffs its services. Ask every provider who performs benefits checks, payer follow-up, and denial work, and how the practice can review that work.
For a self-billing practice, managed billing adds a new expense. For a practice already paying a biller, compare the full package with that existing fee. At an illustrative $10,000 in monthly net collections, Bomi’s required 4% fee is $400; the modeled EHR and all-session AI bring the subtotal to $516 before third-party charges.

Switching from SimplePractice to TherapyAppointment
If you are switching from SimplePractice, separate clinical records, future appointments, and unpaid insurance claims in the migration plan. An exported chart does not establish that outstanding billing work has transferred.
Before canceling, ask the destination for an import checklist and a sample reconciliation. Verify client count, signed notes, attachments, active treatment plans, future appointments, payer details, and open balances. Keep a written cutover date and an owner for old claims. Do not assume payer enrollments, payment credentials, or every document format move automatically.
Run a complete workflow with a synthetic client: intake, scheduling, documentation, billing, payment review, and export. Record missing fields and manual steps. A successful demo is more useful than a long feature checklist, especially when you depend on custom forms or staff permissions.
